How can I obtain/request an issuer statement?

Am I a CHESS Sponsored Holder or an Issuer Sponsored Holder?
If you have entered a sponsoring agreement with a broker (or another CHESS sponsor) then you are a CHESS Sponsored Holder - also referred to as a Broker Sponsored Holder. If not, then you are an Issuer Sponsored Holder. CHESS Sponsored holdings are identified by a Holder Identification Number (HIN) which starts with an X while Issuer Sponsored holdings can be identified by a Securityholder Reference Number (SRN) which start with an I.
I do not appear to be receiving any correspondence in connection with my Avexa shares. Why?
Your address may not be up to date on Avexa's share registry Computershare's records. If not, please contact Computershare on toll free (within Australia) 1300 850 505 to provide correct details.
I do not know my HIN/SRN. How can I find out?

What is a SRN?
The SRN is the identifying number which links your personal details to your individual investment. It is made up of the letter I followed by ten numbers e.g. I0001234567. The SRN provides you with additional security for your investments. This number is very important and should be recorded and kept secure as it will need to be quoted in all dealings and communications with the share registry or a broker. If you misplace your SRN you will need to contact Computershare. A confirmation of your holding stating your SRN will be sent to you.
I have received an issuer statement from Computershare Investor Services Pty Ltd but it does not app
If you have any broker issued shares, these will not appear on your issuer statement. You will need to request a CHESS statement from your broker.
How do I transfer securities?
If you wish to transfer securities into the name of another person or party, you may arrange an off-market transfer, also known as a standard transfer. You will need to download and complete a Standard Transfer form from Avexa's share registry's website. Forward the completed form to the relevant Computershare office. If you have any queries on completing the standard transfer please call Computershare on toll free (within Australia) 1300 850 505 for assistance. Please note, the Australian State government tax, Stamp Duty, on transfers of quoted marketable securities executed on or after 1 July 2001 has been abolished. If you have further queries in relation to Stamp Duty please contact your nearest Australian State Office of State Revenue or financial advisor.
How do I buy/sell securities and shares?
If you wish to trade securities and shares on the Australian Stock Market, you will need to contact a licensed stockbroker to arrange the transaction. Alternatively if you have already registered with Computershare, you can opt to sell your issuer sponsored securities through the Visitor Trade facility on the Computershare website. There are many broking providers that you may choose from, most of whom can be found in the Australian Yellow Pages or on Australian Stock Exchange (ASX). If your securities are broker sponsored and you are wishing to make a trade on these securities you must contact the sponsoring broker.

I am moving house and need to change my address. How do I do this?
If you have issuer sponsored shares, you can contact Avexa's share registry, Computershare, on toll free (within Australia) 1300 850 505 and you will be sent a change of address form or alternatively, you can obtain this form from Computershare's website. If you have CHESS/broker sponsored shares, you must do this through your broker.
How do I notify Avexa of any changes of personal details such as my address, bank details, name etc?

Will I be charged if I require a re-print of information already provided from Computershare?
Most print outs of information will be free, however as from 1 February 2006, some items will incur a fee. Please see the attached schedule of fees from Computershare to see if fees apply.
How secure is the information I provide to Avexa's share registry Computershare Investor Services?
Avexa understands that privacy is important to you. Avexa and Computershare are subject to the requirements of the Privacy Amendment (Private Sector) Act 2000 (Privacy Act), which governs, among other things, how organisations handle personal information. Personal information such as your name, address and details of the shares you hold, but not your banking instructions or SRN/HIN, are included on a public register as required under Chapter 2C of the Corporations Act 2001 (Cth). This personal information may be disclosed to certain people or organisations outside Avexa including Avexa's contractors (such as Computershare), brokers, printing companies, market researchers, and to the Australian Taxation Office, law enforcement agencies and others as required by law. If some or all of your personal information is not collected, Avexa may not be able to administer your shareholding. You may access all your personal information by contacting Computershare on toll free (within Australia) 1300 850 505. For security reasons you will be asked to quote your SRN/HIN. Some requests may have to be put in writing.
